Continuing Medical Education Application Process

  1. All entities applying for CME activities should be oriented by CME Division personnel. If this is the first time to apply for credits, the person applying should meet with the Program Director or request a telephone interview.
     

  2. Application for CME is available through Ms. Amariliz Torres in the following e-mail: amtorres@psm.edu. You may also call 787-840-2575 x 2115. The filled application must be received at least 90 days prior to the date of the activity for MDs, and other health care professionals. These dead-lines are in accordance to regulations established by the Puerto Rico Board of Medical Examiners (TEM) and the Puerto Rico Office of Regulation and Certification of Health Professionals (PR-ORCHPS).
     

  3. Application should have the following documents:

    • Curriculum Vitae (or Biographica Sketch) of speakers

    • Copy of the Activity Educational Program

    • Completed Curricular Design

    • Pre-Test / Post-Test
     

  4. Fill up applicable areas in the “Letter of Agreement”.
     

  5. All Speakers and/or personnel involved in the CME program, should sign a “Disclosure Letter” form, which points out the nature of relation with the entity offering educational grants (if applies). Information should be disclosed to CME activity participants. CME Division will deliver this document to the speaker(s) and /or personnel involved, by mail, e-mail or fax, enclosed with a letter informing the title of activity, place, date, and time of the activity and specific academic objectives.
     

  6. Activities will be attended by PSM/CME Division personnel that have the responsibility for registration, distribution and recollection of evaluation forms, and to prepare and deliver CME certificates if requested.
     

  7. Evaluations will be analyzed, and results will be informed to speakers and the sponsoring entity. This information may be used as a tool for the planning of future activities.
     

  8. Semester planned activities will be revised by the
    Steering Committee for CME (CELC) on a
    monthly basis.
